Northwest AHEC has partnered with North Carolina AHEC and Unite Us to deliver an educational series aimed at educating Healthcare Professionals about a new tool for providing holistic patient centered care.
The series is designed to promote NCCARE360, a cutting edge statewide network built to coordinate health care and human services organizations using technology which enables person-centered approach for delivering care in North Carolina. NCCARE360 helps healthcare and human service professionals electronically connect patients to community resources. The tool is available in all 100 counties across North Carolina.
Northwest AHEC is excited to partner with Unite Us in bringing NC Care 360 to NC providers as a tool to address the non-medical drivers of health in North Carolina. A regionally focused, statewide educational effort is underway to ensure that anyone in the human services arena can be knowledgeable about both this innovative platform, as well as some of the community service providers in their regions.
Northwest AHEC began a series of 5 monthly webinars in September 2020 and has seen robust attendance and audience engagement with 95 providers attending the first two webinars in the series. Each webinar focuses on a single domain (housing, food insecurity, employment, transportation and personal safety).
Continuing education credit is available, there is no cost the events, and recordings are available to those not able to attend the live lunch time events.
