Collaborative Care Model (CoCM) Training Series

New Modules Released!

We are excited to announce the release of modules 18-20 in the CoCM Training Series.

Module 18: Engaging Patients in Care

Speaker: Monica Harrison, MSW, LCSW

Patient engagement and ownership of their care plan are key aspects of the first core principle, Patient-Centered Team Care.  This course orients BHCMs in the important aspects of engaging patients in Collaborative Care, the role of stigma in patient engagement and how the model decreases this barrier.

Module 19: Working in a Rural Setting

Speaker: Diane Powers, MA, MBA

This webinar describes what we know about rural health disparities, the challenges and opportunities in implementing Collaborative Care in rural settings, adaptations that have been implemented by successful rural health organizations, and the implementation experiences and clinical outcomes of 23 rural clinics with the model as part of a two-state initiative.

Module 20: Deploying an Effective Collaborative Care Team

Speaker: Monica Harrison, MSW, LCSW

This webinar focuses on laying foundational knowledge around Collaborative Care team roles and the CoCM structure of the team.  Sustainability and treatment-to-target measurable outcomes are tied to having an effective team.  The presenter reviews the different components and types of team interactions, and how to ensure timely and appropriate communication.  Knowing these elements is essential in introducing CoCM and the team to the patient population receiving this service.

The North Carolina Department of Health and Human Services (DHHS) has partnered with NC AHEC to provide educational and practice-based support to primary care practices interested in implementing the Collaborative Care Model (CoCM) -- a team-based, interdisciplinary approach to deliver evidence-based diagnoses, treatment, and follow-up care for patients with mild to moderate behavioral health needs.

Our CoCM Training Series is comprised of modules which demonstrate how to apply this model to implement this modality. We will measure learner knowledge with an evaluation assessment tool following the completion of the series. Modules will be released on a monthly basis and will provide you with the initial steps to implement CoCM and the tools for ongoing support. Contact hours, CEUs, NASW-NC Contact Hours, AMA PRA Category 1 Creditâ„¢, and NBCC will be offered.
