Application for $5,000 grants to support Covid-19 vaccination in primary care
NC AHEC and the UNC Prevention Research Center invite North Carolina primary care practices to apply for grants of up to $5,000 to support the delivery of Covid-19 vaccines. Practices should apply through this link: https://unc.az1.qualtrics.com/jfe/form/SV_86Ag2KHXWGKw4qa no later than September 17, 2021.
Funded practices will receive:
- Up to $5,000 reimbursement to support Covid-19 vaccine delivery
- Practice support from NC AHEC coaches
- Project support from trained UNC student volunteers (depending on availability)
Practices should propose projects that meet the needs of their community. Eligible expenses include:
- Staff time or materials to support outreach and scheduling assistance for unvaccinated patients.
- Staff time for a Covid vaccination “drive” or related efforts to increase vaccine access.
Funds may not be used to purchase food, gifts/incentives, or non-Covid-19 vaccination related materials or equipment. Eligible practices:
- Are pediatric, family medicine, or internal medicine practices
- Currently deliver Covid-19 vaccines
- Can provide data on the number of Covid-19 vaccine doses delivered during the project period
Practices in areas with lower vaccination rates will be given special consideration. After project completion, practices will be required to submit:
- A W-9 form
- A brief description of project activities
- A record of funds spent on project activities, including relevant receipts
- CVMS or EHR report with aggregate number of administered Covid-19 vaccines
Funded practices will be reimbursed after project completion after submitting data, completed forms, and receipts. These grants are made possible with funding from the Centers for Disease Control and Prevention.
