CoCM AHEC

The North Carolina Department of Health and Human Services (DHHS) has partnered with NC AHEC to provide educational and practice-based support to primary care practices interested in implementing the Collaborative Care Model (CoCM) -- a team-based, interdisciplinary approach to deliver evidence-based diagnoses, treatment, and follow-up care for patients with mild to moderate behavioral health needs.

 

Our CoCM Training Series is comprised of 10 modules which will help to demonstrate how to apply this model to implement this modality. We will measure learner knowledge with an evaluation assessment tool following the completion of the series. Modules will be released on a monthly basis and will provide you with the initial steps to implement CoCM and the tools for ongoing support. Contact hours, CEUs, NASW-NC Contact Hours, AMA PRA Category 1 Credit™, and NBCC will be offered.

New Module Released!

 

The newest module in the CoCM training series is Module 3: Putting Collaborative Care Principles into Practice: Planning for Clinical Practice Change. Participants will learn how to apply the core principles of Collaborative Care for a successful implementation. After teams lay a foundation for Collaborative Care, the next steps are for the team to put the principles in practice as they get ready to launch Collaborative Care. These steps involve identifying the Collaborative Care team members, planning clinical workflows, developing a registry strategy, and defining critical process and outcome measures to track the program’s success. This webinar will cover the steps necessary for a program to take before they launch Collaborative Care.

 

Be sure to check out previously released modules, including:

  • Module 1: Collaborative Care Model (CoCM) Rationale and Evidence
  • Module 2: Laying the Foundation for Collaborative Care Through Practice Transformation
