North Carolina Medicaid would like to better understand how Clinically Integrated Networks and Advanced Medical Homes screen beneficiaries for unmet social needs, process data, and make referrals for interventions.

 

Why is your organization's participation important?

Integrating social resource interventions into care planning is incredibly important for improving health!

This project will inform NC Medicaid's efforts to leverage the statewide Health Information Exchange to collect and exchange screening data on unmet resource needs.

What does participation involve?

Your organization should determine your best point of contact---someone who is familiar with your policies and practices related to social determinants of health.  then, the point of contact would...

1. email the project lead, Elizabeth Tomlinson, PhD, RN to schedule your interview

2. Complete the brief (~10 minutes) project survey via Qualtrics

3. Complete a ~1 hour interview via Zoom
