Letters going out to Medicaid Direct beneficiaries eligible for Tailored Care Management

Your patients who are eligible for Tailored Care Management will receive a letter between now and November 17th.

These notifications will:

• Explain the member's ability to opt-out
• Identify the member's assigned tailored care management agency
• Give direction on how the member can CHANGE their assigned tailored
care management agency by CONTACTING THE LME

Even though the launch of Tailored Care Plans has been delayed until April 1, 2023, Tailored Care Management and assignment will take effect December 1, 2022.
