Carolina Access Practices: New 2 Party Agreement with CCNC needed

For those serving NC Medicaid Direct beneficiaries, be on the lookout for a new primary care case management (PCCM) agreement from NC Carolina Community Care Networks within the next 1-2 weeks.

This agreement must be signed and in place in order to receive quality measure results (such as well child visit and immunization data) from CCNC. The email will come from CCNC through either hi@mail.concordnow.com or hello@mail.concordnow.com.
