NWAHEC Awarded NC Medicaid Grant to Support 5 Online Learning Activities

The Northwest AHEC Practice Support Team was awarded a grant from NC Medicaid supporting five different online self-paced learning activities.

Developed in collaboration with the Continuing Professional Development team, these modules focus on Screening and Intervening for Unmet Social Needs, and Reverse Behavioral Health Integration. By using the steps and tools included in the different modules, clinical and behavioral health team members can implement new evidence-based workflows and care delivery strategies to serve their patients and clients. These modules are free to take, all include credit hours, and can be completed on your own time with self-paced online learning.

Behavioral Health Reverse Integration Online Series

The integration of behavioral health care into primary care is now widely considered an effective strategy for improving outcomes for patients with physical and behavioral health conditions. Reverse integration of primary care into behavioral healthcare is another form of integration that produces improved quality of care and patient outcomes. These modules focus on the reverse integration process and provide details on how to implement this new model of care. Participants will learn the benefits of this model of care, how to use electronic health records (EHRs) to improve continuity of care in an integrated setting, how to integrate physical health assessments, the financial aspects of integrated care, as well as other clinical and operational aspects of providing this model of care. Screening and Intervening for Unmet Social Needs

Everyone deserves to live their healthiest life. Unmet social needs impact health outcomes more than clinical interventions. The National Academy of Medicine, Center for Medicaid and Medicare Services and the World Health Organization have long advocated that screening for these needs be integrated into clinical settings but barriers exist. This course provides tools, resources and recommendations to support practice teams to screen and facilitate intervening to meet these needs. Learn more here.
