NC*Notify Version 4+  is suitable for organizations who want to receive more targeted notifications for a subset of their patients and leverage the NC HealthConnex platform for downstream processing and coordination of follow-up activities. Notification transfer methods include Direct Secure Messaging (DSM) and Secure File Transfer Protocol (sFTP) with the added option of near real-time notifications.

In addition to the COVID-19 notifications, the new alerts that are now available include:

  • High utilizer alert – This notification helps providers identify frequent visitors to emergency departments (two or more visits in 90 days or less) or patients at high risk for readmission (four or more admissions in 12 months).
  • Dental alerts – Providers are alerted when patients visit the emergency department for dental care.
  • Care team change alert – Providers are informed when a new organization has subscribed to their patient.
  • Diabetes diagnosis alert – Providers are alerted upon a new diabetes and/or pre-diabetes diagnosis for patients they are monitoring.
  • Chronic care management alert – Providers receive an alert when a patient meets the Centers for Medicare and Medicaid Services' chronic care management services criteria.

How NC*Notify Works

Here's an example of how NC*Notify works.

  1. A patient goes to the hospital or urgent care.
  1. The patient has x-rays and other tests.
  1. The hospital or urgent care sends information about the patient's visit to NC HealthConnex.
  1. NC HealthConnex notifies the patient's provider about the visit.
  1. The patient's provider makes a follow-up appointment for any additional care.

Version 2 of NC*Notify will no longer be supported after Tuesday, May 11, 2021. To prepare for these upcoming changes, each subscriber will need to review and make necessary changes to ingest the new notification result files adhering to the NC*Notify V4 Technical Specifications and NC*Notify V4+ Technical Specifications before Tuesday, May 11, 2021.

Please contact the SAS NC HealthConnex Help Desk if you have any additional questions regarding these changes.
