Curious about the Collaborative Care Model whether it's right for your practice and your patients?
What is the evidence that it works (clinically and financially)?
Perhaps you have implemented the model and have questions or ideas to share with your colleagues.
Our colleagues at Southern Regional AHEC are hosting a Learning Collaborative for primary care providers, behavioral health professionals and anyone else who has implemented or is considering the Collaborative Care Model for integrating behavioral health in a primary care setting.
Those in an independent primary care practice, federally qualified health center or rural health clinic may find the collaborative especially helpful for meeting some of the behavioral health needs of their patients.
The Collaborative takes place on 6 dates in 2024, beginning on January 18th; all from 12:30-1:30PM
