New ICD-10 Codes to Document COVID-19 Vaccination Status

In response to the ongoing COVID-19 public health emergency, the Centers for Disease Control and Prevention’s (CDC) National Center for Health Statistics (NCHS) is implementing three new diagnosis codes into the International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) for reporting COVID-19 vaccination status, effective April 1, 2022.

 

The codes were presented by the National Center for Health Statistics (NCHS) at the Sept. 14-15, ICD-10 Coordination and Maintenance Committee meeting, so they are not listed in the 2022 ICD-10-CM code book.

The new ICD-10-CM codes for reporting COVID-19 vaccination status are:

Diagnosis Code Description
z28.310

Unvaccinated for COVID-19

may be assigned when the patient has not received at least one dose of any COVID-19 vaccine. 

z28.311

Partially vaccinated for COVID-19

may be assigned when the patient has received at least one dose of a multi-dose COVID-19 vaccine regimen, but has not received the full set of doses necessary to meet the Centers for Disease Control and Prevention (CDC) definition of “fully vaccinated” in place at the time of the encounter.

z28.39 Other underimmunization status
