State Designated Rural Health Centers (SDRHC) offer high quality primary health care services to all residents. Office of Rural Health defines an SDRHC as a health care safety net organization that is a 501(c)3 non-profit, community-owned organization with an active board that has as its primary mission to provide primary health care services to those residing in its community. SDRHCs must be located within communities that are both rural and underserved and must currently be delivering primary health care services. 

We were excited to hear about our practices nominations for the SDRHC of Excellence awards. It's great to see such dedication and excellence across Rural Health Centers in NC.

SDRHC of the Year: Grace Clinic of Yadkin Valley

This SDRHC shows excellence across all areas, especially working with AHEC coaches and collaborating with ORH. This SDRHC has shown excellence in using local resources and collaborating with other partners to respond to health care gaps in their community. The SDRHC has shown excellence in innovative strategies to promote health equity and inclusion.

This SDRHC has shown excellence in blending behavioral health services fully or partially within the primary care practice. We’re looking for a candidate who demonstrates excellence or marked improvement in providing behavioral health services that are fully or partially integrated within the primary care practice, with a focus on uninsured and underinsured patients.
