The Office of Rural Health (ORH) and North Carolina Area Health Education Center (AHEC) invite you to participate in no-charge educational offerings to learn more about NC CARE 360 and Screening/Intervening for Unmet Social Needs.

Modules from human service organizations covering 5 priority domains include impactful client stories.

ORHCollab (2)Activity 1 shares an overview of statewide initiatives aimed at addressing Social Determinants of Health and Activity 2 facilitates Screening and Intervening for Unmet Social Needs in your practice by providing online access to evidence-based resources and practical tools. (If you are a current CA II provider, you are eligible for reimbursement for this work under the Healthy Opportunities Screening and Referral Program through at least June 2021.)

Activities 3-7 are recorded webinars from human service organizations and representatives from NC Care 360. While the featured organization may not specifically serve your region, they include case studies that tell meaningful client stories, and the role of the NC Care 360 platform.

All online lectures are available via your computer or mobile device, on-demand.

Who would be interested in these activities: Physicians, advanced practitioners, nurses, behavioral health specialists, social service professionals, medical assistants, community health workers and others.
